Job Description:

The Division of Community Psychiatry is seeking an experienced, licensed, psychiatric social worker for Program Director of Case Management and Homeless Services which includes Case Management, the SSI Outreach Project, PATH Outreach, and the Safe Haven Shelter. These programs provide services for adults who have serious and persistent mental illness, many who are homeless in Baltimore City.

  • Maintains quality of mental health services and meets clinical requirements
  • Develops and monitors program budgets as well as grant activity
  • Provides clinical leadership, guidance and consultation to programs
  • Provides administrative and clinical supervison to all staff, some aspects of which may be delegated to qualified professional staff
  • Coordinates services with other agencies.

Qualifications:

  • An advanced degree (Masters) from an accredited college or university in Social Work, with a LCSW-C.
  • A minimum of five years progressively responsible experience performing health care or outreach services, preferably with homeless or severely mentally ill populations. In addition, a minimum of three years supervisory/management experience is required.
  • Proficient knowledge of psychotherapy, psychosocial, clinical psychology, psychological treatment modalities, state and federal guidelines/regulations for the assigned treatment population is required. General knowledge of pharmacology, organizational behavior and relevant psychiatric medications is required.

Under the supervision of the Director, assist with the oversight, development, management, maintenance, operation and quality assurance of Ethel Elan Safe Haven

Competencies :

Work requires a comprehensive understanding of mental health and social services acquired by completion of a High School Diploma, course work in mental health or related field and at least two years of successful work experience in a supervisory capacity. Associates Degree in a Human Services field is preferred.

Knowledge :

Work requires knowledge of the needs of homeless persons diagnosed with mental illness, the mental health service system, shelters, and emergency services.

Skills :

Must possess high level of analytical ability to gather and interpret information in relation to the residents’ needs and the program operates.

Must possess clinical skills necessary to oversee the programs’ operation and maintenance

Must possess interpersonal skills to interact with consumers, families, core services agencies, human service programs and other public and private entities.

Budget Responsibilities :

Must work closely with Director to ensure proper use of funds. Monitors and keeps written records of resource usage. Oversees the delivery of services to ensure that records meet standards set forth by funding sources. Helps ensure that consumers gain access to services that help meet their emotional, financial, somatic, and environmental needs.

Work frequently requires high levels of mental and physical fatigue when overseeing resident care and program maintenance and safety

Social Work Manager

GENERAL POSITION DESCRIPTION.


The Case Manager coordinates care planning for clients through assessment, service facilitation, referral, advocacy and follow up. The Case Manager works collaboratively with clients, client’s support system, partners and providers to coordinate delivery of quality health care. Provides all case management functions that assist clients’ with the diagnosis of HIV/AIDS. Completes routine and grant specific case management reports. Assesses plans, implements, coordinates, monitors and evaluates the options and services available to meet individual patient resources needs. The Social Worker/Case Manager uses communication and available resources to promote quality and cost effective outcomes.

Exhibits and promotes a standard of excellence in the performance of all duties and interactions with patients, co-workers, and outside contacts.


ESSENTIAL JOB DUTIES

1.
Provides direct clinical social work services to patients across the life cycle.

2. Responsible for overseeing all activities of the Social Work Department.

3. Coordinates health and human resource services for referred clients.

4. Supervises the social work staff; monitors department budget.

5. Screens patients referred by physicians to determine economic and/or environmental risk.

6. Provides direct support services, including but not limited to: assessment of eligibility for FHCB transportation assistance; assistance with pharmacy co-pay/facilitates medication vouchers as per protocol; and presumptive eligibility screening, and such other health resources available for the medically indigent.

7. Assessment of eligibility for state and city assistance programs.

8. Initiates applications for pharmacy assistance and state assistance program.

9. In collaboration with primary care services, develops individual client case management plan.

10. Monitors and tracks client progress and compliance. Provides follow up services as determined appropriate.

11. Provides crisis intervention when the client experiences sudden changes in physical or emotional health or when he/she is a risk to self or others. Coordinates interventions to serve and protect the client’s well-being.

12. Promotes social work and related services. Initiates mental health services improvements.

13. Performs other duties as assigned.

Requirements
EDUCATION/WORK EXPERIENCE



LCSW-C required. MSW Degree required. Five plus (5+) years of clinical social work experience in a community health or public health setting with under-served populations. Knowledge of community resources.



ENVIRONMENT/WORKING CONDITIONS

Work is performed in a clinic environment. Involves frequent contact with staff and the public. Work may be stressful at times. Contact may involve dealing with angry or upset people.


PHYSICAL\MENTAL DEMANDS

Varied activities including standing, walking, reaching, bending, lifting. Requires full range of body motion including handling and lifting patients manual and finger dexterity and eye-hand coordination. Requires standing and walking for extensive periods. Requires corrected vision and hearing to normal range. Requires working under stressful conditions or working irregular hours.

ADDICTIONS COUNSELOR

SPECIAL POPULATIONS SERVICES

 

I. MAIN PURPOSE OF THE POSITION

Working as part of the bSAS Special Populations Services Team, the Special Populations Services Addiction Counselor will assist in the provision of a comprehensive continuum of substance abuse treatment and outreach services to homeless individuals and other special populations in their communities and other places where the client may frequent. Within the guidelines put forth by the Maryland State COMAR Regulations, counselor will provide triage, assessment services, formulate treatment plans, provide individual and group counseling, collect urine specimens, and make referrals to treatment programs and follow-up to determine whether connections have been made. The addiction counselor will make referrals to other adjunct agencies as indicated by the assessment of clients’ needs. The position will also act as a liaison between bSAS and other agencies interaction with special populations.

II. MINIMUM QUALIFICATIONS (Education, Experience and Licensure)

Possession of Bachelor’s Degree in Social Work, Human Service, Psychology or related field and five years of professional experience in substance abuse treatment and experience working with special populations (i.e. homeless substance abusers, mentally ill, women and children, etc.), must be licensed by the Maryland Board of Professional Counselors or other specified Health Occupations Act. Acceptable certifications/licensures include:

· Certified Professional Counselor – Alcohol and Drug (CPC-AD), or

· Certified Associate Counselor – Alcohol and Drug (CAC-AD), or

· Certified Supervised Counselor – Alcohol and Drug (CSC-AD)

III. MAJOR DUTIES AND RESPONSIBILITIES:

1. Assist in the development, implementation and provision of substance abuse treatment services for drug affected/dependent persons and their families.

2. Provision of addiction services at various homeless shelters and other locations where special populations are identified in Baltimore City.

3. Serve as liaison between bSAS, substance abuse treatment providers, Hands In Partnership (HIP) and other agencies providing services to the homeless and special populations.

4. Maintain relevant statistical data; prepare progress reports and other correspondence as needed.

5. Provide regularly scheduled shelter based groups focused on orientation; alcohol and drug education; group counseling; family counseling; and gender specific groups.

6. Provide outreach services to Baltimore City residents. Outreach services will include: engaging homeless and other special populations on the street; at soup kitchens; at homeless encampments and other areas designated by the Mayor of Baltimore, the Baltimore City Health Department, the Office of Homeless Service and bSAS as areas in need of outreach services.

7. Position provides substance abuse treatment/outreach services on a flexible work schedule to include days, evening and weekends.

8. Provide technical assistance to community organizations.

9. Evaluate project activities to determine effectiveness and to achievement of goals.

10. Perform related work as required.

IV. SPECIAL REQUIREMENTS OR CONDITIONS:

• Extensive knowledge of substance abuse problems and treatment issues.

• Knowledge of principles and practices of program administration, design and management.

• Ability to work effectively with community organizations, community leaders, service providers and consumers.

• Excellent written and verbal communication skills.

• Willingness to provide services in emergency situations and at special events.

V. SALARY RANGE $35-$40K

III. General Summary:

The Special Projects Coordinator will provide critical staff support to the President and CEO by performing a range of tasks relating to ongoing operations and specific projects. The Coordinator will coordinate activities within the agency and with external partners.

  • Managing timelines to ensure that projects are executed in a coordinated manner within the agency and with external partners
  • Developing and implementing special projects as assigned
  • Conducting research and data analysis
  • Writing reports, briefings, proposals and position statements
  • Administrative follow-through, including drafting letters and developing presentations
  • Reviewing and summarizing complex documents and reports
  • Attending external meetings as a representative of BMHS
  • Other duties as assigned

V. Knowledge, Skills and Abilities Required:

The successful candidate will possess strong organizational, analytical and problem-solving abilities. Excellent interpersonal, research, written and oral communication skills are required. Masters degree in Public Health, Public Administration or other relevant graduated degree and relevant.
